You have the right as a patient to receive an explanation of the billing and policies regarding payment of fees.
We will be happy to discuss the facility charges with you in advance for your procedure. We ask that you bring your insurance cards with on the day of surgery. As a courtesy, we will bill your insurance carrier or governmental agency directly for the Surgery Center charges.
The patient is responsible for any upfront deductibles and coinsurance payment. A deposit is required at the time of admission which represents the portion of the facility charge not reimbursed by your insurance company. If the charges are rejected by your insurance company, you or the responsible party are expected to pay the balance with 30 days.
If you require special financial arrangements, please contact our office prior to our surgery to discuss alternative methods of payment.
We will be happy to accept Visa, MasterCard, Discover Card, American Express, and cash or check for form of payments that need to be made.
The Surgery Center fees cover the use of the facility only and not include laboratory, pathology or your surgeon’s services or anesthesia services. You may expect to be billed separately for those services.
